Arlyn visited with Dr. Darius Enario, endocrinologist, at Maayo Hospital in Mandaue City, Cebu. The reason for the visit was to review her current diabetes medications and modify as necessary. Dr. Enario referred Arlyn to have full blood workup and to submit herself to have an electrocardiogram, or ECG, performed. Dr. Enario also requested an abdominal ultrasound to look at the liver and gallbladder.

Arlyn visited with Dr. Imee Chu Lim, OBGYN, at Maayo Hospital in Mandaue City, Cebu. The reason for the visit was to conduct her annual OBGYN visit. Dr. Lim ordered a transvaginal ultrasound.

Arlyn visited with Dr. Imee Chu Lim, OBGYN, at Maayo Hospital in Mandaue City, Cebu. The reason for the visit was to discuss the report and analysis from the transvaginal ultrasound. The report indicated a septated cyst in the left ovary. The cyst is a septated cyst with a hypo-echoic border and color Dopper flow representing the corpus leteum. Since the corpus leteum is normal, no concern or action is required.

"Septated" refers to a structure divided into smaller, distinct parts by thin walls or membranes called septa. It is commonly used in medical imaging to describe cysts, tumors, or organs (like the uterus or gallbladder) that have internal partitions.

The corpus luteum is a temporary, yellow endocrine gland that forms in the ovary from a ruptured follicle after ovulation during each menstrual cycle. Its primary function is to produce large amounts of progesterone and moderate estrogen, which prepare the endometrium for potential pregnancy, and it persists for about 14 days before degenerating if fertilization does not occur.

Arlyn visited with Dr. Darius Enario, endocrinologist, at Maayo Hospital in Mandaue City, Cebu. The purpose of the visit was to discuss the findings from the ECG, abdominal ultrasound and lab work.

The ECG report indicates a sinus rhythm with anterior wall ischemia. Sinus rhythm is the normal, healthy heart rhythm where electrical impulses originate in the sinoatrial (SA) node, typically resulting in a regular rate of 60 to 100 beats per minute (bpm) in adults. An anterior wall is the forward-facing or frontal side of an organ, cavity, or structure in the body, such as the abdominal wall, stomach, or heart. Key examples include the abdominal wall layers (skin, muscles, fascia), the heart's anterior surface, or the vaginal wall. It commonly serves to protect, provide support, or act as a surface for attachment. Ischemia is a critical condition defined by reduced blood flow to tissues or organs, causing oxygen starvation and potential cell death. Primarily caused by vascular blockages like plaque buildup (atherosclerosis) or blood clots, it can cause severe dysfunction in the heart, brain, or limbs. Anterior wall ischemia is a serious, often life-threatening condition where the heart's front wall (supplied by the left anterior descending artery) receives insufficient oxygenated blood. Dr. Enario was to refer Arlyn to see a cardiologist for further evaluation.

The abdominal ultrasound indicates the presence of multiple lithiases, or gallstones in the gallbladder, the largest being 1.8 cm in size. This is consistent with previous findings from California. Lithiasis refers to the formation of stony concretions (calculi) in the body, most commonly in the urinary tract (kidney stones) or gallbladder (gallstones). These stones are often composed of mineral salts and can cause intense pain, nausea, and obstruction.

Dr. Enario would like to refer Arlyn to see Dr. Paul Gacrama, gastroenterologist, to review the findings with Arlyn.

Arlyn visited with Dr. Paul Gacrama, Gastroenterologist, at Maayo Hospital in Mandaue City, Cebu.

After reviewing the images from the 2/2/2026 scan, it was determined that the stones are not blocking any gallbladder ducts. Since Arlyn is not experiencing any pain or discomfort from the gallstones, it was determined that no action will be taken at this time.

Elective surgery is available to remove the gallbladder. By doing so, the gallstones issue is removed.

If the gallstones become lodged in the gallbladder ducts, this will cause pain and discomfort. Surgery will be required to clear the ducts. It must be noted that if this happens, gallbladder removal cannot be performed at the same time. The removal of the gallbladder must occur at a later date. Suggest performing the removal of the gallbladder in the future before a gallstone becomes lodged in the gallbladder ducts.

March 30, 2026

Arlyn visited with Dr. Barbra Destajo, Cardiologist, at Maayo Hospital in Mandaue City, Cebu. After visiting with Dr. Destajo, she recommended a stress test with ECG and 2D Echocardiogram with Doppler.

Arlyn visited with Dr. Barbra Destajo, Cardiologist, at Maayo Hospital in Mandaue City, Cebu. This visit occurred shortly after Arlyn completed the Stress Test with ECG. Dr. Destajo was present during the stress test.

According to the stress test report, dated April 7, 2026, there was occasional premature ventricular contractions occurring singly noted at state I and stage II with more frequent premature ventricular contractions at the start of stage III and noted couplets and non-sustained three beats. Ventricular Tachycardia (VTACH) noted during peak exercise. There was also exaggerated blood pressure response to stress.

Ventricular tachycardia (VT) is a fast, often dangerous heart rhythm (>100 beats/min) originating in the ventricles, causing ineffective pumping. Often triggered by heart damage (scarring, heart attack), it causes palpitations, dizziness, chest pain, or fainting. Immediate treatment includes electrical shock or medication; long-term, it may require a defibrillator (ICD).

Immediate attention is required to prevent the possibility of a future heart attack. Dr. Destajo forwarded Arlyn's case to Dr. Keith Chan, cardiologist, for further evaluation.

Arlyn visited with Dr. Keith Chan, Cardiologist, at Chong Hua Hospital - Mandaue in Mandaue City, Cebu.

After reviewing Arlyn's test results, Dr. Chan concluded that Arlyn would need to undergo an angiogram. In addition, if the angiogram indicates the need for a stent, an angioplasty can be performed. The angiogram would need to be performed as soon as possible.

Arlyn indicated that she was experiencing gastrointestinal issues with diarrhea. With this information, Dr. Chan stated the angiogram would need to be delayed for one week to allow the gastrointestinal to pass.

April 23, 2026

Arlyn was admitted to UCMed Hospital in Mandaue City. The angiogram and possible angioplasty was scheduled for April 24, 2026 at 8:30 AM.

On 4/24/2026 9:10 AM, the angiogram was started. It ended at 9:19 AM. Here is a summary of the test's findings.

On 4/24/2026 9:34 AM, the angiogram was started. It ended at 9:54 AM. Here is a summary of the procedure. A Medtronic Resolute Onyx RX Drug-Eluting stent was deployed to the significant mid to distal left main artery lesion. No complications occurred as a result of the deployment.

Arlyn visited with Dr. Darius Enario, endocrinologist, at Maayo Hospital in Mandaue City, Cebu. The purpose of the visit was to discuss an update on her medications. Dr. Enario wants to have another set of lab tests performed. But he did update Arlyn's prescriptions. They are now:

Dr. Enario wants Arlyn to have some labs performed on Monday, May 11, 2026. He also wants Arlyn to visit with Dr. Enario on Tuesday, May 12, 2026 to discuss the results of those labs.

Arlyn's weight on this day is 69.6 kgs (153.4 pounds).

Arlyn visited with Dr. Barbra Destajo, Cardiologist, at Maayo Hospital in Mandaue City, Cebu. This visit is a follow up with a cardiologist two weeks after the angiogram/angioplasty. Dr. Destajo reviewed Arlyn's medications and tweaked the heart meds slightly (see below). Dr. Destajo would like to see Arlyn in 90 days.

Dr. Enario was pleased with the progress made by Arlyn. He removed two medications and added a new one. The following is a list of medications modified by both Dr. Destajo and Dr. Enario:

Arlyn visited with Dr. Darius Enario, endocrinologist, at Maayo Hospital in Mandaue City, Cebu. The purpose of the visit was to review Arlyn's progress with Ozempic and see if she can tolerate a higher dosage. Arlyn indicated that the Ozempic was tolerated and she was ready for the next level.

Dr. Enario would like to see Arlyn between July 3 and July 8, 2026 to review her tolerance to the higher dosage of the Ozempic. There is no need to do labs.

July 6, 2026

Arlyn's weight on this day is 67.3 kgs (148.3 pounds).

Arlyn visited with Dr. Darius Enario, endocrinologist, at Maayo Hospital in Mandaue City, Cebu. The purpose of the visit was to review Arlyn's progress with Ozempic and see if she can tolerate a higher dosage. Arlyn indicated that the Ozempic was tolerated and she was ready for the next level. Dr. Enario prescribed a higher dosage and removed the Metformin from her after-breakfast dose. Arlyn will take Metformin in the evening. This is to address his concern about the higher dosage of Ozempic and hyperglycemia. The new prescriptions are as follows:

Dr. Enario would like to see Arlyn during the first or second week in August to track her progress with the Ozempic. Labs are required prior to this visit.

Arlyn visited with Dr. Darius Enario, endocrinologist, at Maayo Hospital in Mandaue City, Cebu. The purpose of the visit was to review Arlyn's progress with Ozempic. Dr. Enario reviewed Arlyn's test results and was concerned with the increase in triglycerides. It increased from 206 (May 11, 2026) to 219 (August 3, 2026). This may be attributed by the increased amount of fruit Arlyn was consuming during the last couple of weeks. As a result, Dr. Enario increased the dosage of the fenofibrate from 145 mg/tab to 250 mg/tab. Arlyn can finish her current supply of fenofibrate before moving on to the new dosage.

Dr. Enario also increased Arlyn's semaglutide dosage. He switched her to the Wegovy brand. The new dosage is 1.7 mg/PFS (milligrams per pre-filled syringe).
